In terms of intense stuff like that, concentrate just on the physical sensation and when thought tries to come back in, just continually keep moving your attention back onto the physical sensation. In terms of Eckhart's and similar teachings, when we are lost in thought and therefore unaware that is ego. When we wake out of thought and know we were just thinking that is conscious awareness.
